IShould you be a captain / owner of any vessel(s) that may not have been surveyed, please print off and fill in the survey form and send it to the alliance. Otherwise, please call the Alliance and leave your name, number, and the associated vessel name, should you opt to be surveyed in person. Either way, the Alliance will confirm both the contact and the information.
